HSSC plans to share the results of this survey, so that you can leverage expertise across the state, share lessons learned, collaborate and potentially build on each other’s work. Together we have a remarkable opportunity to gain national distinction by engaging the strengths of multiple partners to turn back this epidemic. It is a pivotal point in time for us to come together to improve the lives of South Carolinians.

We need your help in disseminating this survey to ensure that we capture a complete picture of the current standing of diabetes and obesity activities within our state. Get the word out! Forward this survey to anyone in South Carolina who may be working on a relevant project.
