You must have completed your one year contract unless you meet one of the exceptions noted on the form.

No refunds will be given for Paid-in-full (prepaid) contracts.

Cancellations will go into effect on the last day of each month. You can verify if your request has been completed by logging into your Club Automation memberhship account.
