Sea Island CATCH Community Survey

Summary
You are be asked to take part in a community research study being done among the Gullah Geechee that live within the Gullah Geechee Cultural Heritage Corridor. The purpose of this study is to learn about people's knowledge, attitudes, and behaviors towards different types of common cancers. You do not have to take part in this study and your participation is up to you. The study is being paid for by a grant from the National Institutes of Health National Cancer Institute and is being done by the First Baptist Church of James Island, SC in partnership with the Medical University of South Carolina. You may not benefit from taking part in this study and we know that your time is valuable; but, we hope that the information we get will help improve cancer outcomes and quality of life within the community.

We have permission from the Medical University of South Carolina IRB to do this study (Pro00090658). By submitting this survey, you are agreeing and giving your consent to take part in it.

Rights of Participants
Your participation is voluntary and up to you. You do not have to do the survey. You have the right to stop at any time. You also have the right not to any answer question(s) on the survey that you do not want to.

Survey Instructions
The survey was made to be easy to complete. It should take about 10-15 minutes. Please give a response to all questions and pick 'Prefer not to say' to any question(s) that you do not want to give an answer to. At the end of the survey, you will be asked if you would like to be entered into a giveaway for a Fitbit Alta HR and also if you would like to take part in a follow-up phone interview with the researchers about your answers.

If you want to take part in the giveaway, you will be asked for your name, address, and telephone number. 4 people from each State will be picked by computer to receive a Fitbit Alta HR. If you are picked, the researchers will contact you, identify themselves, and verify the address where the Fitbit will be mailed.

If you would like to take part in the follow-up phone interview, you will also be asked for your name and telephone number. 4 people from each State will be selected by the researchers to do the phone interviews. If you are selected, the researchers will call you, identify themselves, and set-up a good time with you to do the interview. This call will take about 30-45 minutes, and you will get a $50 Walmart giftcard by mail for doing it.
